UMBILICAL CORD–DERIVED MESENCHYMAL STEM CELLS (UC-MSC) THERAPY USING THE VINSKI PROTOCOL IN A STRUCTURED CARE PATHWAY FOR REFRACTORY AUTOIMMUNE DISEASE: A PROSPECTIVE SINGLE-CENTRE OBSERVATIONAL CASE SERIES
Abstract
Mesenchymal stem cells, particularly those that are umbilical cord-derived, are a stem cell therapy approach with immunomodulatory potential in addressing autoimmune disease as a major cause of chronic disability and morbidity. An integrated multi-system mesenchymal stem cells treatment program employed a multi-system, cross-domain customization approach referred to as the Vinski protocol, which combined closed-system umbilical cord-derived mesenchymal stem cells therapy with complementary, individualized clinical strategies. A single tertiary regenerative medicine center served as the research site for this prospective observational case series (Celltech Stem Cell Centre in South Jakarta, Indonesia). The sample consisted of adult patients with refractory autoimmune disease. Each patient received one or more therapy cycles. Most patients were previously exposed to conventional treatments. Umbilical cord-derived mesenchymal stem cells were manufactured from screened allogeneic umbilical cord tissue using a good manufacturing practice-aligned closed system. Outcomes were defined as the change in disease-specific activity indices and patient-reported outcomes. Measurements were taken at baseline and at three, six, and 12 months. Secondary outcome measures were collected to assess steroid-sparing effects, inflammatory biomarkers, and safety. The therapy resulted in clinically meaningful reductions in disease activity scores, improved fatigue and pain scores, and decreased oral corticosteroid requirements in a significant proportion of patients. No therapy-related serious adverse events, neoplasia, or opportunistic infections were observed during the follow-up window. In this single-site study, umbilical cord-derived mesenchymal stem cells therapy delivered within a structured multidisciplinary care pathway was safe and feasible and resulted in relief of symptoms for adults with refractory autoimmune disease. Definitive conclusions will require controlled trials, but this research contributes to the rationale and the potential of this clinical protocol and its multi-system, system-wide interventions based on a framework of customization to needs. Customized co-interventions to facilitate optimal UC-MSC therapy, per the Vinski protocol, can represent a novel approach that advances the science and theory of the treatment of refractory autoimmune diseases. The treatment approach, therefore, warrants continued research.
